Applications for Minors
Effective November 1, 2004, parental consent forms must be notarized.
All applicants under the age of 18 MUST appear in person at the Embassy and complete Form DS-11. All applicants under 14 years of age MUST appear together with both parents. In the event that both parents cannot attend, the absent parent may execute a notarized statement of consent. It is essential that this statement of consent from the absent parent is notarized; no exceptions will be made to this rule. This statement must also contain an unequivocal consent to passport issuance for the child (or children). This statement of consent may take the form of a DS-3053, or a sworn affidavit.
The parent who can attend may then bring this statement of consent into the Embassy, along with the child (or children) and photographic identification from the absent parent which contains the absent parent’s signature.
